Tersin Injection contains Terlipressin, a synthetic vasopressin analog used primarily in the management of acute bleeding due to esophageal varices and in the treatment of certain conditions related to low blood pressure. Terlipressin works by constricting blood vessels (vasoconstriction), which increases blood pressure and reduces bleeding. It is commonly used in critical care and emergency settings for patients who require vasopressor support, particularly in the setting of severe bleeding or shock.

Active Ingredient:

  • Terlipressin (as acetate): A prodrug that is converted into its active form, lysine vasopressin, in the body. It causes vasoconstriction by binding to vasopressin receptors in blood vessels, leading to an increase in systemic vascular resistance and blood pressure.

Indications: Tersin Injection is typically used for the following conditions:

  • Acute bleeding from esophageal varices: Tersin is used in cases of severe bleeding caused by esophageal varices, typically seen in patients with cirrhosis of the liver. By constricting blood vessels, terlipressin reduces portal venous pressure and helps control the bleeding.

  • Hypotensive shock: Particularly in septic shock or shock resulting from other causes, where the blood pressure is critically low and other vasopressors have failed to improve circulation.

  • Acute kidney injury (AKI) in cirrhosis: Tersin is used in patients with hepatorenal syndrome, a condition where the kidneys fail due to liver cirrhosis.

  • Intraoperative hypotension: In certain surgical settings, Tersin may be used to stabilize blood pressure when vasoconstriction is needed.

Dosage and Administration: Tersin Injection is typically administered intravenously (IV) in a hospital or clinical setting by a healthcare professional. The dosage and duration of treatment depend on the specific condition being treated and the patient's response to therapy.

  • For acute bleeding from esophageal varices: Initial dose is usually 1–2 mg every 4–6 hours, which may be adjusted based on clinical response.

  • For hypotensive shock: The initial dose is often 1 mg every 4–6 hours or as required, with adjustments based on blood pressure monitoring.

  • For hepatorenal syndrome: Terlipressin is often administered as 1–2 mg every 4–6 hours until improvement in renal function is observed or the maximum treatment duration is reached.

The duration of therapy depends on the patient's response to the medication and the underlying condition. Tersin should be administered with caution in patients with cardiovascular conditions, as it can lead to significant increases in blood pressure.

Side Effects: Common side effects may include:

  • Bradycardia (slow heart rate).

  • Headache.

  • Abdominal cramps or discomfort.

  • Nausea or vomiting.

  • Facial flushing or a feeling of warmth.

Serious side effects (though rare) may include:

  • Hypertension (high blood pressure): Terlipressin can significantly increase blood pressure, leading to complications like stroke or heart failure in susceptible patients.

  • Cardiac arrhythmias: Due to its vasoconstrictive effects, terlipressin can cause changes in heart rhythm, especially in patients with pre-existing heart conditions.
