Indications:
Zoldonat is used for the treatment of several conditions related to bone health, including:

  • Osteoporosis: To increase bone mass and reduce the risk of fractures in postmenopausal women and men with osteoporosis.
  • Paget's disease of bone: To treat bone remodeling disorders, which leads to abnormal bone formation and breakdown.
  • Hypercalcemia of malignancy: To treat high calcium levels in the blood caused by certain types of cancer.
  • Bone metastases: To prevent skeletal-related events (e.g., fractures) in patients with cancers such as breast cancer and multiple myeloma that have spread to the bones.

Mechanism of Action:
Zoledronic acid, the active ingredient in Zoldonat, is a bisphosphonate that works by inhibiting osteoclast-mediated bone resorption. Osteoclasts are cells that break down bone tissue. By reducing bone breakdown, zoledronic acid helps increase bone density and strength, reducing the risk of fractures. In cancer-related conditions, it helps lower calcium levels in the blood and prevent bone complications due to metastases.

Administration:
Zoldonat is administered intravenously (IV) by a healthcare professional. The typical infusion time for 4 mg is about 15 minutes, depending on the condition being treated. The dosing schedule varies:

  • For osteoporosis, it is typically given once a year.
  • For Paget’s disease, a single dose may be administered, and additional doses may be given based on response.
  • For hypercalcemia of malignancy or bone metastases, doses are given more frequently depending on clinical needs.

Contraindications:

  • Hypersensitivity to zoledronic acid or any of its components.
  • Severe renal impairment (creatinine clearance less than 35 mL/min).

Side Effects:

  • Dental checkup: A dental examination should be done prior to starting therapy, especially for cancer patients, to reduce the risk of osteonecrosis of the jaw.
  • Hydration: Adequate hydration should be maintained before and during the infusion to help prevent kidney-related side effects.
  • Electrolyte levels: Calcium and other electrolytes should be monitored regularly during treatment.

Storage:
